JUST TRYAN IT, INC. agrees to use my information solely in furtherance of the mission of JUST TRYAN IT, INC. and agrees not to disclose, transfer, release or sell such information to any third party for any purpose. I understand that the information provided by me will be used in turn at the discretion of JUST TRYAN IT, INC. I understand that the information I provide may be edited (primarily for length and not specific content) by JUST TRYAN IT, INC. and used (either spoken or in print) in any and all educational, promotional and/or marketing materials, including but not limited to, website displays, brochures and posters. I further understand that the information I provide may be used in fundraising efforts by JUST TRYAN IT, INC. and/or its participants. I acknowledge that there is no compensation to me, my family or my child.This Consent to Participation and Release of Information shall continue in perpetuity. I understand that I may withdraw this Consent to Participation and Release of Information at any time by providing sixty (60) days prior written notice to JUST TRYAN IT, INC. If I withdraw this Consent to Participation and Release of Information, I agree that JUST TRYAN IT, INC. may continue to use any pre-printed educational, promotional and/or marketing materials containing my information, but that no materials will be created following the effective date of my withdrawal of this Consent to Participation and Release of Information.

I, hereby, on behalf of myself (or) the minor child for whom I am the parent or legal guardian, consent to and give permission for JUST TRYAN IT, INC. to use my/our photos and videos or any and all photos and videos taken in connection with my/our/their appearance and participation in any JUST TRYAN IT events. I acknowledge that JUST TRYAN IT, INC. has full ownership rights to the images I give them and are taken at or in connection with JUST TRYAN IT events, and to the photo and video productions it creates, and that my and/or my child’s appearance, may be transmitted or otherwise exhibited, in whole or in part, throughout the world, without limitation as to time, in any medium, and by any means, method or device now or hereafter known, by JUST TRYAN IT, INC. as many times as it wishes, without financial compensation to me and/or my child or further authorization from me for either me or my child. Consent to this Photo Release shall continue in perpetuity.
